Parents initially complete an Application for Enrolment form, then complete a formal School Enrolment Form once eligibility for enrolment is confirmed.
Application for Kindergarten 2026 enrolments are now open. If your child was born between 01 July 2021 – 30 June 2022, they are eligible for Kindergarten 2026. Please complete the application form above and email to hollywood.ps@education.wa.edu.au along with other documents as noted below. Applications for Kindergarten 2026, close on Friday 25 July 2025.
Similarly, if your child was born between 01 July 2020 – 30 June 2021, they are eligible for Pre-primary 2026.
A copy of your child’s birth certificate or passport must be provided during the enrolment process. Students will be placed in the year of study corresponding to their age group. A copy of the student’s Australian Immunisation Register, available online from Medicare, is also required.
Evidence of proof of residency within the local intake area must be provided at the time of enrolment. This should include either a copy of the rates notice or lease/rental agreement, proving the applicant is residing at the address. The Principal also reserves the right to request a Statutory Declaration in support of claims made.
